Building Value-Based and Clinically Integrated Networks

CLIVE Solutions has a knowledgeable team with decades of combined expertise in developing, implementing, and supporting integrated care programs, including health home, CCBHCs, as well as promoting and providing evidence-based training and requirements for providers and their staff. We have been on a long journey and learned many lessons working with different vendors and approaches to collect data, implement population health strategies, value-based care analytics, and report on outcomes.

CCBHC Model Preparation, Readiness, and Implementation

CLIVE Solutions has experience in preparation and checklist-based readiness for CCBHC model creation and implementation. CLIVE support includes the implementation of the CCBHC model in concert with state guidelines, measures, and certification standards. CLIVE provides consultation on all facets of developing the CCBHC Model, including CCBHC required services, care coordination, data collection, reporting, outcome measure tracking, certification standards, PPS rate setting, target populations, and evidence-based practices.

Substance Use Disorder System Redesign

CLIVE Solutions team helped substance use disorder treatment providers redesign their services, including implementing the American Society of Addiction Medicine (ASAM©) criteria, development of actuarily sound service rates, comprehensive training, and use of evidence-based practices.
